If you are a new patient and want to print the paper work needed for your first visit, you may use the link at the bottom of this page labeled 'Dr. Brian Moraes- New Patient Forms'.

Clicking on the link will direct you to a new web site called 'EZ Files'. On the EZ Files page, there will be a small box at the bottom of the page that will prompt you to wait approximately 10 seconds and will count down on your screen. Once this countdown reaches 0, the box will read 'Download Files Now'. Click this download box and choose to open the file (do not save the file).

You will need a PDF program of some type, such as Adobe Reader, to open the file. If you require the free version of Adobe Reader, please click the link at the bottom of the page labeled 'ADOBE READER' and follow the installation instructions on the Adobe website. 

Once you have opened the new patient forms file, you can print out all the forms needed for your first visit by clicking the printer icon. You can return to our web page by using your browser back arrow. Although you may type in your information on these forms, we do require a signature in ink, therefore, faxing these forms back to our office is not possible.

Please remember, you still need to call our office first to make an appointment.

Also, please be aware that as of 1/1/2009, our office will be charging a $25 cancellation fee if you miss your appointment without at least 24 hours prior notice or if you are more than 15 minutes late for your scheduled appointment.
